What does the Home 1st Reablement service do?
Our reablement service is provided to keep you independent at home for as long as possible and to prevent you needing to go into hospital.
We have a ‘stand back’ approach to helping you regain your ability to look after yourself in your own home. We will work with you to identify some achievable goals that can be met within a six-week period. We may provide you with some equipment that can help with this or make suggestions about how you could do something differently. We will always encourage you to do as much as possible for yourself. You might have been unwell recently, or recovering from an injury or going home from a period in hospital.
How long will we be working with you?
Our team will work with you for up to six weeks. We will provide regular updates to you about your progress. You may not require our support after just a few days.
How much will it cost?
In most cases reablement in your own home will not cost you any money. If your reablement at home extends beyond six weeks you will be assessed financially, and you may have to contribute to the cost of your care.
