The service is available to people over 18 registered with a Milton Keynes General Practitioner (GP). The team accepts referrals from any health or social care professional for example GPs district nurses therapists intermediate care team leaders or any other community or hospital-based professional. Referrals are made through Single Point of Access Intermediate Care Services.
