Reablement Support Worker
This role undertakes work to aid the reablement of people and enables them to achieve optimal independence. Reablement support workers develop reablement plans, undertake non-complex assessments and provide flexible programmes and interventions of therapeutic activities. It is a beneficial role helping people reach and maintain their independence.
Senior Reablement Support Worker
This role undertakes specific support work to aid the reablement of people and enable them to achieve independence without support of others. Senior reablement workers also undertake medium complexity assessments and reviews of an individual's needs. They also provide flexible programmes and interventions of therapeutic activities directed by Registered Physiotherapists and Occupational Therapists.
Home1st Coordinator
The role of a Home1st Coordinator is to provide leadership and team leadership support to the team, and to work directly with people to carry out an assessment of care needs and risks.
