What does Short Breaks do?
Milton Keynes Short Breaks Service is based at Mathiesen Road which supports individuals over the age of 18 who have profound and multiple learning disabilities, complex health needs, autism and who need support with behaviours of concern (see our Services Model)
The service has wheelchair accessible transport which is used for community activities. The service supports people to increase their presence in our community with access to support to entertainment venues, leisure facilities and local walks. As well as community activities the service offers in-house activities such as art, music, sensory sessions, and physiotherapy. All activities provided aim to promote independence and choice.
The service provides an essential overnight and daytime respite for family carers providing a much-needed break from their caring roles. The staff work closely with health and social care professionals, each person using the service will have a detailed individual support plan to meet their physical and emotional needs.
How to access this service
People can access the service following an assessment by Adult Social Care and referral by their Social Worker. The service also accepts referrals from professionals within the Continuing Health Care Team and supports people who are funded in this way.
