To access the CTALD health services, Adults with a learning disability need to be registered with a GP in Milton Keynes and have a Milton Keynes residential address.  

Referrals

Referrals can be made directly or with your permission by someone who works with you. To discuss a referral either you or a family member can contact the CTALD Duty Nurse on 07852 474373 or the ACCESS Team on 01908 253772.

To discuss a referral or for advice about transitioning from Children's Services to CTALD contact your social worker, GP or case manager.
